Contact the Hotel directly at (727) 593-6001 and refer to our “MBAA” group. A block of rooms has been assigned to this meeting. Deadline for Reservations is September 6, 2006 (call Joyce Lay if you cannot make reservations before Sept. 6th).
Please fill out the attached survey sheet and mail it with a check for $70.00 for Members, $45.00 for Spouse and each Guest/Child over the age of 10, and $25.00 for each Child under 10 years of age. Checks should be made payable to MBAA-District Southeast and mailed to Joyce M. Lay, 111 Busch Drive, Jacksonville, Florida 32218.
Other:
- For those members not returning the attendance survey in advance of the meeting, an additional $10.00 for Members and $5.00 for Spouses or each Guest/Child, will be assessed.
- There is a 48-hr hotel cancellation policy in effect. This Policy states that you must cancel your hotel reservations at least 48 hours prior to the meeting.
- Cancellation of meeting registration must be communicated in writing before September 29, 2006. If this is not done the registration fees will be forfeited to the District.
Several suppliers belonging to our organization have expressed an interest and desire in sponsoring a Hospitality Room or assisting in another event. If you are interested in helping, please contact David Voykin or Joyce Lay. All sponsors will be recognized during the meeting and Saturday night’s Banquet.
